After the closing date, we have contracted with a Medical Records Custodian, Shoreline Records Management, Inc. Authorized requestors will request their records from Shoreline by accessing their website and filling out the online Medical Records Release Form. When Shoreline receives the form back, they will notify the requestor of the processing fee.
They charge $25.00 per chart. Once payment is received, they will send a secure link via email to the requestor, who can then access and download the chart, or patients may request a CD mailed to their address for an additional $10.00 fee.
Please note that Shoreline will only release a copy of the full chart.
